Welcome Back / Reactivating Students
Welcome back! We're glad you've decided to return to finish your degree or continue taking classes for professional development.
If want to register for classes after an absence of one quarter or more, you will need to be reactivated through the Graduate School. To do that, please fill out this Reactivation Form and return it to the Office of Student Services and Graduate Studies. You can scan the form (pdf format only) and send it to edplssgs@osu.edu, or you can fax it to 614-292-7900. Whichever option you choose please type the information into the form so that it is easy to read in case we have to contact you.
For those who are returning for post degree professional development - once your reactivation form has been received we will forward it to the Graduate School so that they can assign you a registration appointment. We will email you information on course registration once that occurs.
For those wishing to reactivate into a degree program - students who have been absent from their program for more than two years must request readmission to continue pursuing their degree. Along with your completed Reactivation Form we ask that you write a new statement of intent explaining your absence and how you plan to complete your degree requirements. If your previous advisor is no longer with the School, we will forward your file and new statement of intent to the Section Chair for your previous program. Doctoral students MUST include a letter of support from your previous advisor when he or she is still an active faculty member. If your advisor or the section chair approves your re-entry into your previous program, we'll fax your updated information to the Graduate School. Again, once you have been reactivated by the Graduate School, we send you an email with information about activating your email and registering for classes.

Some students just want to come back and take classes for professional development without being readmitted to a degree program. If this situation applies to you, in addition to filling out the above reactivation form, we will ask you sign a waiver letting us know that you understand that you are being reactivated only to take classes and are not being readmitted to a degree program. Once we have both the reactivation form and the waiver we will forward your completed form to the Graduate School and email you with registration information once you have been assigned a registration window.
PLEASE NOTE: If you have a "Do not Release" hold on your records(noted with the closed "window shade" on your Student Center), you will have to come in person with a picture ID to reactivate.
