To apply to the Dietetic Internship, an application packet must be completed and submitted by February 15, 2008. For applicants who are applying for the MS/DI program this application packet is different from the application packet submitted for admission to the graduate school.
The components of the Dietetic Internship Application Packet are as follows:
1. Letter of Application
Your letter of application should be typed or handwritten neatly and should not exceed one page in length. Include career goals, recent experiences, and professional goals for a supervised practice experience not covered in the application.
2. Completed Supervised Practice Program Application Form and $100.00 application fee (a check made payable to The Ohio State University).

3. Declaration of Intent form or Verification Statement.
Obtain either the Declaration of Intent form or Verification Statement from your Didactic Program Director. A Declaration of Intent form should be submitted by applicants who have not yet completed necessary coursework but intend to do so. A verification Statement should be submitted by applicants who have already graduated with a baccalaureate degree and completed the requirements for the Didactic Program in Dietetics as approved by the Commission on Accreditation/Approval for Dietetics Education (CAADE) of the American Dietetics Association. Applicants who submit a Declaration of Intent form and are accepted will be required to submit who submit Verification Statement before the Internship Program begins.
4. Three letters of reference.
Please submit 3 separate letters of reference as part of your Dietetic Internship Application Packet. Be sure that the individuals writing these letters address your qualifications for a supervised practice program. While not mandatory, you may choose to use the Recommended Form (MS Word document) developed by the American Dietetic Association and Dietetic Educators of Practitioners Practice Group.
5. A transcript from each college/university attended.
Include an official transcript from each of the post-secondary schools that you have attended. If the transcript from the school you are presently attending does not include grades from fall semester/quarter, a copy of your fall semester/quarter grade report should also be included in your Dietetic Internship Application Packet. Applicants who are accepted will be required to provide a transcript that shows degree completion before entering the Program.

The Dietetic Internship at The Ohio State University has been granted Accreditation by the Commission on Accreditation for Dietetics Education of The American Dietetic Association, 120 South Riverside Plaza, Chicago, IL 60606-6995.
The Department of Human Nutrition follows ADA guidelines, which indicate that the applicant must enclose a self-addressed, stamped postcard if he or she wishes to be notified of receipt of application.
Incomplete applications will not be reviewed by the Dietetic Internship Selection Committee. Therefore, all materials required to complete the application packet must be received by the deadline.
The Department of Human Nutrition maintains a policy of not returning application materials to the applicant, once the application has been accepted.
The Department of Human Nutrition does not require a personal interview as part of the application procedure. However, a brief 15 minute telephone interview may be requested.

Applicants must have completed the baccalaureate Degree and academic requirements of the Didactic Program in Dietetics (DPD) set by The Commission on Accreditation for Dietetics Education prior to entering the program. Admission to the Dietetic Internship is on a competitive basis. Selection of Dietetic Interns is based on evidence of: conceptual ability, overall academic preparation, interpersonal skills, leadership potential, self-direction, ability to work independently, and ability to perform under pressure.
Applicants to the Dietetic Internships (DI) must participate in computer matching. Applicants should request instructions and a mark/sense card to prioritize their DI or Ap4 preferences. Applicants should request this material from any ADA-approved Didactic Program Director.
