The application materials listed below are to be used by Ohio State graduate students who wish to enter a Master's or Doctoral program in the School of Physical Activity & Educational Services.  All application materials for the new program should be sent to A100 PAES Building; 305 West 17th Avenue Columbus, OH 43210.

Note:  Current or previous OSU graduate students do not reapply to the Graduate School through the same graduate admissions process and no application fee is required in such cases.

Your application will be comprised of the following:

• Click here for an internal (within PAES programs or Master's to PhD) Transfer Application
• PAES Master's to PAES PhD student will also need to complete the Master's Appendix G Form.
• If you are transferring from a non-PAES grad program into PAES, you will need the Graduate School's Transfer of Graduate Programs Form.
• A Statement of Academic Intent giving academic and professional goals related to the program you wish to enter
• Three (3) letters of recommendation written within the last 12 months for master's program.  Three (3) letters are preferred for application to doctoral program.
• A copy of your Graduate School application materials from your former program or department
• Test Scores:

  GRE scores (within 5 years) from the Educational Testing Service (ETS). You may register for the exam through the GRE website. Only scores from the general exam are needed.  The OSU Office of Testing is an official testing site:  614-292-2241. (Most successful applicants admitted score at least 1000 for a combined verbal and quantitative score and 4.0 on the writing section.)

Application deadlines for transferring into PAES are the same as those for students who have not enrolled in a graduate program at Ohio State.  If you find that the file from your earlier program/degree no longer exists, it will be necessary for you submit copies of transcripts from any schools attended other than Ohio State.
