Orientation & Mobility (O&M) – Application Checklist

Application Open Date: November 1st annually
Application Deadline: April 1st annually
Summer Start Program


  • Select Your Application
    • Complete the Online TK20 Form
      • Go to the form and login. Complete the following steps if you do not already have an account.
    • To create an account, follow this form
      • Scroll to the bottom of the page and click on “Admissions”
      • Scroll to the bottom again and click on “Create a free account”
    • Once logged in, click on the “Create New Application” green button
      • From the drop-down menu, select the “OSU-License Only-Orientation and Mobility (O&M) Program” and click on the “Next” button.
      • Complete the form with your information.
      • Click the “Submit Application Now” green button.
  • Resume: Submit electronic copy by uploading document to the online application. Must be typed and no longer than 2 pages. Recommended guidelines are:
    • Academic background (major, minor)
    • Academic accomplishments (scholarships, fellowships, awards, etc.)
    • Relevant experiences with children/adolescents
    • Other Involvement, service, work experiences
  • Statement of Intent: Submit an electronic copy by uploading your document to the online application. Must be typed, double-spaced, no longer than 4 pages, and include your first and last name and date of birth on every page. Recommended prompts are:
    • Discuss meaningful experiences (both positive and negative) you had with children/adolescents. Reflect upon what you learned from these experiences and their impact on your teaching practices.
    • What contributions after obtaining this license?
    • Convey how you have displayed a commitment to learners who have a lived experience that is different from yours. Reflect upon what you learned from these experiences.
  • Transcripts: Please upload transcript documents via TK20. Include all coursework; undergraduate, graduate, etc.
  • Additional Documents: Upload any documents you feel are relevant to the program application. This may be your teaching license, additional transcripts, etc.
  • Letters of Recommendation: 2 letters are required. Your references should submit 1 page letters electronically to tl-academicservices@osu.edu before the application deadline.

After Applying

Upon acceptance into the TVI program, you will be emailed by the TLAS Office. You will then need to apply to the OSU Graduate School as a graduate non-degree student. That application can be found here.

Important Application To-dos

  • A bachelor’s degree is required to apply to this program.
  • Applicants are advised to keep copies of all application materials submitted to OSU for their own records.
