Returning from leave of absence
If you took a Leave of Absence from Ohio State and are now ready to return, please follow the steps below. Former Ohio State students do not need to reapply to the university.
In order to initiate your reactivation, please complete this online form. This form should be submitted prior to the beginning of the semester for which you are returning. Once the form is submitted, you will receive both a confirmation e-mail and follow up communication regarding your reactivation status.
Please note, the ability to declare a certain major or pre-major is contingent upon whether or not you meet the minimum GPA requirement for your selected program.
To schedule an appointment with an academic advisor, please call our office at 614-292-9261. When scheduling your appointment, please mention that you are a former student who is planning to return.
Returning from dismissal
If you are dismissed from the University, you will be eligible to apply for reinstatement after two (2) semesters away from Ohio State. You can request an appointment with an EHE academic advisor to discuss the reinstatement process and plan for success at Ohio State. The reinstatement petition is an opportunity for you to provide the review committee with context about the causes of your academic difficulty, an analysis of the steps you are currently taking or plan to take to improve your academic performance, and evidence of any progress you have already made. Each student case is reviewed individually, and the review committee will consider reinstatement to students who have provided sufficient evidence that they will be successful if allowed to return to the program.
